Your rights
- To be treated with respect and without discrimination or reprisal.
- To receive the information needed to give informed consent, and to refuse or withdraw consent at any time.
- To know the identity and role of everyone involved in your care.
- To privacy and confidentiality.
- To be free from all forms of abuse, neglect, and harassment.
- To care in a safe setting.
- To be told about physician financial interest in advance.
- To know the center's advance directive policy.
- To voice a grievance without fear and to receive a written response.
- To be informed about the fees you may be responsible for.
Your responsibilities
- Give us accurate health history.
- Follow the instructions your care team gives you.
- Arrange a ride home, as required for your procedure.
- Arrive on time.
- Tell us when something changes, such as a new medication, illness, or symptom.
