Requesting your medical records.
You have the right to see and receive a copy of your health records. Here is how to request them.
How to request records
To request a copy of your records, complete a written records-request form and send it to us by mail, fax, or in person.

ID requirements
You will need to show a valid, government-issued photo ID that matches the name on the records request. If you are requesting records on behalf of someone else, you will also need documentation showing you are legally authorized to receive them (for example, a signed authorization form or proof of legal guardianship).
Fees
We will tell you the amount, if any, before processing your request.
How long it takes
If we keep your records electronically, Texas law requires us to provide them to you electronically, in the form you request if we can produce it, within 15 business days of a written request.
