Your protected health information may be subject to electronic disclosure.

Pulse Surgery Center creates and maintains health information about you in electronic form. Your information may be disclosed electronically. For example, to your other treating physicians and to Pulse Healthcare System practices for your treatment, to your health plan for payment, and for our health care operations.

Except as permitted or required by state or federal law, we will not electronically disclose your protected health information to any person without a separate written authorization from you or your legally authorized representative for each disclosure. Authorization is not required for electronic disclosures made to another covered entity for purposes of treatment, payment, health care operations, or performing an insurance or health maintenance organization function, or as otherwise authorized or required by state or federal law.
