Advance directives.

An advance directive is a document that tells others what care you want if you cannot speak for yourself. Texas recognizes three kinds.

• Directive to Physicians and Family or Surrogates. Also called a living will. It states the care you want or do not want.
• Medical Power of Attorney. It names a person to make health care decisions for you if you cannot.
• Out-of-Hospital DNR. It tells emergency responders and staff outside a hospital not to attempt resuscitation.

Bring a copy of your advance directive if you have one. We will scan it into your record. Having one, or not having one, will not change the care you receive.
